Our medical surveillance program is risk based and reviewed by an OHP (Occupational Health Practitioner) / OMP (Occupational Medical Practitioner) depending on the risk as well as the guidelines provided by the relevant medical counsels.  All Medical Examinations are done in accordance with the new construction regulations of 7 February 2014 and the Occupational Health and Safety Act 85 of 1993.   All medicals are done according to the SASOHN position statement.

TERMS AND CONDITIONS OF SERVICE

GENERAL

The client is responsible to provide Solidity Occupational Health (Pty) Ltd with accurate and correct information. Solidity Occupational Health do not accept any liability or responsibility if the client supplied fraudulent information. Fraudulent information is a crime; we are responsible by law to report such activities. All tests, files, certificates are the property of Solidity Occupational Health until all accounts are settled. All legal fees to collect any account will be for the clients account. Employees must have a valid ID, passport or driver’s license present when conducting the medical. It is the responsibility of the Company to ensure the validity of an employee /   employee’s ID/ passport or driver’s license.

Solidity Occupational Health does not take responsibility when an employee did not disclose any conditions or illnesses that might impact the status of his/her fitness.

The client is responsible to provide Solidity Occupational Health (Pty) Ltd with a signed fully completed request for medical examination for risk based medicals or Annexure 3 for construction medicals.

MEDICAL FITNESS CERTIFICATES

All OHS medical examinations are conducted in accordance with our SASOHN, SASOM guidelines and OMP recommendations.

Fitness Certificates will be valid for 12 months; employers do need to take note of recommended re-test dates as well as expiry dates on the certificate to ensure that employees are tested in accordance with our recommendations.

When employees are excluded from certain working areas (noise zones, respiratory zones, heights, driving) it is an OHS recommendation and should be taken seriously as the employee with certain exclusions is unfit to work in those working areas (or unfit to do those duties).

REFERRAL LETTERS

Referral letters will be issued to the employee.

It is the responsibility of the employer to ensure that the employee returns to our clinic with written feedback once the referral letter was actioned by the relevant health practitioner.

DRUG SCREENING

Drug Screening is a HR related test. Should an employee have a non-negative result, the result will be given through to the employer to make a decision on whether they want to do confirmation testing at a pathology laboratory.

AUDIOMETRY (HEARING TESTING)

It is the responsibility of the employers to specify whether the employee requires a baseline audio test (this is a requirement when the baseline audio result for the employee is not available to the employer). A baseline audio can only be done once in an employee’s life. Employers need to make annual comparisons to ensure that deviations from baseline audios are monitored. An employee can claim from the compensation fund when the deviation is more than 10% from his baseline audio results. The compensation fund documents can be obtained from the Department of Labor (www.labour.gov.za/DOL/documents/forms/)

All employees must be in a noise free area for a period of 8-16 hours before testing is performed . (SANS 10083)

SPIROMETRY (LUNGFUNCTION)

Employees with any of the following conditions would need to disclose the condition as they should not complete a lung function test:

Pregnant, breastfeeding,

Uncontrolled Hypertension – Stage 2

Heart attack in the past 3 months,

Eye surgery in the past month

Heart condition for which they are taking medication,

Epilepsy,

Diabetes with BP above 130/90, taking specific medication

ON-SITE MEDICALS

The Client shall be invoiced for all employees as per quote accepted.

The client shall be responsible on day of testing to ensure the health and safety representative or dedicated employee is available to ensure that all employees on the list to be tested are called for the tests.

The client be responsible to provide Solidity Occupational Health with a clean noise free working space with access to an electricity point for our electrical equipment. The client be responsible to provide Solidity Occupational Health with a clean, private working space (one office room) as well as ablution facilities where clinic establishment may proceed.

Minimum of 50% of signed quote will be invoiced should less delegates not arrive as booked.

PAYMENT

The Company acknowledges indebtedness to Solidity Occupational Health to the sum of the medical examinations that will be undertaken, plus interest at a rate of 10.25% per annum a tempora morae (the “outstanding amount”) on late payments. The Company agrees to pay the outstanding amount within 7 days. Where a 30-day account is in place the Company agrees to pay the outstanding amount within 30-days from receipt of statement. Should the Company fail to make payment on the due date/s, Solidity Occupational Health shall demand the outstanding amount from the debtor together with legal costs; failing which Solidity Occupational Health, without notice, shall be entitled to apply to the relevant Magistrate’s Court for judgement against the Company for the outstanding amount including legal costs – the Company agrees to judgement in this regard. No variation in terms of this agreement, novation, or cancellation by mutual consent shall be of any force or effect unless reduced to writing and signed by both Solidity Occupational Health and the Company.
